Your son/daughter is eligible to participate in a School/Religious Education sponsored activity requiring transportation to a location away form the parish premises.  This activity will take place under the guidance of employees from St.Henry Parish.

If you would like your child(ren) to participate in this event, please complete, sign and return this form to your Director of Evangelization & Catechesis (St.Bernard Parish) or your Coordinator of Religious Education (St.Henry Parish).

Please note that this form can either be printed out, completed and returned to the above referenced person or you can complete this form on-line and submit it electronically.  If completed on-line, please note that the inclusion of your name on this form constitutes a legal substitution of your signature.

I hereby consent to participation by my child named below, in the St. Henry & St. Bernard Youth Groups _____________.  I understand that my child will be under the supervision of the designated parish employee on the stated date(s).  I further consent to the conditions stated in the event description, participation in said event, including the method of transportation (if applicable).

As parent/legal guardian you remain fully responsible for any legal outcome which may result from any personal actions taken by your child.  Your signature fully releases the parishes of St. Bernard and St. Henry, its pastors, its Director of Evangelization & Catechesis, Coordinator of Relgious Education, the Diocese of Madison and its Bishop from any and all liability..
